1. The MyBox Home, MyBox Auto, and MyBox Health, MyBox Senior Health and MyBox Vida insurance policies have an initial three-year term, and the premiums are paid on a monthly basis. No surcharges will be applied for splitting the payment. From the fourth year, the MyBox insurance policy will be renewed automatically every year. 2. Adeslas Salud y Bienestar is a service accessible to persons insured under the Adeslas policy who are over 18 years old. This service and the digital health services are not available in Asturias, Navarre or the Basque Country.

4. Cover available for insured persons up to 62 years of age only, whose premium will be calculated considering the insured amount and the age of the insured person at that time. Coverage for illness of cancer; acute myocardial infarction, by-pass and transplant; stroke; kidney failure; degenerative diseases (Alzheimer's, Parkinson's and multiple sclerosis) and vital organ transplant are mutually exclusive. If any of these conditions occurs, coverage for the others will be terminated and 50% of the pay-out provided for death or absolute permanent disability cover will be made, the remaining 50% being held exclusively to provide cover for death and absolute permanent disability, death by accident and absolute permanent disability by accident, which will continue to be in force for that amount. A 180-day exclusion period will apply to cover for cancer, acute myocardial infarction and other serious illnesses. The coverage for absolute permanent disability is for in the event that the insured person suffers an irreversible physical condition, beyond his/her control, that prevents him/her from carrying out any type of paid work either as an employee or a self-employed person. The condition of permanent total disability does not apply to situations in which the insured person is only prevented from carrying out his/her usual occupation but is able to perform or carry out another job, profession or trade (permanent total disability). Customers over 62 and up to 67 years of age may only take out MyBox Vida life insurance with death cover.
5. Double payout due to an accident applicable in cases where death or permanent total disability occur within two years of the accident.
6. The second medical opinion service in the event of serious illness with MyBox Vida will be provided for three years after the contract date, as long as the policy is in force and premium payments are up to date. The additional service will be provided by the company Teladoc-Health Care Management, S.A., or, failing that, by another company with similar characteristics.
